Best evidence summary for skin care in patients with hematopoietic stem cell transplantation complicated by graft-versus-host disease

Objective To search,evaluate and integrate relevant evidence of skin care for patients with hematopoietic stem cell transplantation complicated by graft-versus-host disease(GVHD). Methods Based on the “6S” pyramid model,the evidence was systematically searched in computer decision support systems,websites of guidelines,relevant professional associations and organizations and comprehensive databases,and evidence of skin care for patients with hematopoietic stem cell transplantation complicated by GVHD were screened from the establishment of databases to May 2022,including guidelines,clinical decisions,evidence summaries,systematic reviews,and expert consensuses. The quality evaluation,evidence extraction and integration of the evidence were conducted by 6 research members. Results A total of 15 pieces of literature were included,including 2 clinical decisions,6 guidelines,5 expert consensuses and 2 systematic reviews. 13 pieces of best evidence were summarized from 4 aspects,including multidisciplinary cooperation,skin assessment,nursing interventions and health education. Conclusion This study systematically integrated the relevant evidence of skin care for patients with hematopoietic stem cell transplantation complicated by GVHD,and provided evidence-based references for the development of relevant guidelines and clinical practice standards. When applying the evidence,nurses are recommended to com-prehensively consider the actual clinical situations and patients’ wishes,and select evidences prudently.

The analysis of the financial toxicity and its influencing factors in prostate cancer patients

Objective The purpose of this study is to investigate the status and influencing factors of financial toxicity(FT) in patients with prostate cancer,and to provide a basic theory for reducing FT levels in prostate cancer patients. Methods With the convenience sampling method,a total of 281 prostate cancer patients were recruited from a hospital in Shanghai from January 2020 to August 2021. A cross-sectional study was conducted by the general data questionnaire,the Comprehensive Scores for Financial Toxicity based on the Patient-Reported Outcome Measures(COST-PROM),the Medical Coping Modes Questionnaire(MCMQ) and the General Self-Efficacy Scale(GSES). Besides,multiple linear regression was used to identify predictors of FT in patients with prostate cancer. Results The total score of COST-PROM was 22.0 (17.0,24.0). Multiple linear regression analysis showed that the factors of the diagnosis of prostate cancer affecting the retirement age,the average income per person in family,the treatment cost in the last 3 months,the dimension of surrender in medical coping style and the level of self-efficacy were the main influencing factors of FT(P<0.001),which accounted for 45.6% of total variation. Conclusion The FT of patients with prostate cancer is in the middle and low level. Clinical staff should use the effective intervention measures to help patients go back to work,estimate their economic conditions for choosing individualized treatment,help them seek more social supports and resources,and guide them to manage problems in positive ways and enhance the level of self-efficacy,in order to reduce the level of FT and improve the quality of their life.

Development of the Perception of Risk of Chronic Kidney Disease Scale for type 2 diabetic patients and the test of its reliability and validity

Objective To develop and evaluate the reliability and validity of Perception of Risk of Chronic Kidney Disease Scale(PRCKDS) for patients with type 2 diabetes mellitus. Methods The item pool was determined by literature review,interview,expert consultation,and pre-investigation,and the formal scale was formed. The newly developed PRCKDS was tested on 420 patients with type 2 diabetes through an online survey. The reliability and validity of the scale were evaluated by Spearman correlation analysis,factor analysis,and Cronbach’s α respectively. Results There were 36 items in the formal scale,with 5 dimensions including susceptibility,severity,beneficence,hindrance and precipitating factors. Spearman correlation analysis indicated that the content validity was good. Confirmatory factor analysis indicated that the model fit was good. The factor loads of 36 items were all greater than 0.45 and within the preset 5 dimensions,indicating that the structure validity was good. The Cronbach’s α of homogeneity is 0.875,and the split reliability is 0.865. Conclusion The PRCKDS established in this study has good reliability and validity,and it can be used as an effective and reliable measurement tool to assess the risk perception of chronic kidney disease in patients with type 2 diabetes mellitus. Different risk perception groups should be differentiated,and individualized strategies should be adopted in the prevention education of chronic kidney disease.
